Advances in cardiovascular and thoracic anaesthesia: Perioperative management of complex cases

Abstract: The management of cardiovascular and thoracic surgeries requires a deep understanding of anaesthesia techniques and perioperative care. With advances in both medical technology and an anaesthetic technique, the management of complex cardiovascular and thoracic cases has evolved, leading to better patient outcomes and reduced complications. The growing prevalence of comorbidities, such as hypertension, diabetes, and obesity, poses additional challenges in the perioperative setting, requiring tailored anaesthetic strategy. Advances in monitoring systems and pharmacological agents have facilitated safer anaesthesia delivery, especially in high-risk patients. The aim of this paper is to review the latest advancements in anaesthetic management for complex cardiovascular and thoracic surgeries, focusing on preoperative optimization, intraoperative techniques, and postoperative care. We explore various strategies for risk stratification, fluid management, and airway management in patients with underlying respiratory or cardiac conditions. In addition, we highlight the role of technology, such as advanced monitoring systems and artificial intelligence, in enhancing decision-making processes. Furthermore, we review the potential benefits of regional anaesthesia techniques and multimodal analgesia in improving recovery outcomes. Lastly, the paper delves into the impact of individualized an anaesthetic plan, guided by patient-specific factors, on long-term health outcomes and recovery times. Despite these advancements, the complexity of such surgeries necessitates careful attention to detail, and there remains a need for further research to refine anaesthesia practices. This review aims to provide anaesthesiologists and healthcare practitioners with updated knowledge on the perioperative management of complex cardiovascular and thoracic cases, emphasizing the critical role of a multidisciplinary approach in achieving optimal outcomes.
