Efficacy of ultrasound guided nerve block versus intravenous tramadol injection for pain relief in patients with fresh lower limb fractures: A hospital based study in Puducherry

Abstract:
Objective: This study aims to compare the efficacy of ultrasound-guided nerve block versus intravenous tramadol injection for pain relief in patients presenting with fresh lower limb fractures in a hospital setting in Puducherry.
Methods: A prospective, randomized controlled trial was conducted over 18 months at Indira Gandhi Government General Hospital and Postgraduate Institute, Puducherry. Patients aged 15-75 years with fresh lower limb fractures were randomized into two groups: Group NB received ultrasound-guided nerve blocks, while Group T received intravenous tramadol. Hemodynamic parameters, pain scores using the Visual Analogue Scale (VAS), and patient satisfaction using a 5-tiered Likert scale were recorded and analyzed.
Results: A total of 108 patients were enrolled, with 54 in each group. Significant differences in mean systolic blood pressure and pulse rate were observed between the groups, with lower values in the nerve block group. Pain scores were significantly lower in the nerve block group at all measured intervals (p<0.001). Additionally, 74% of patients in the nerve block group reported higher satisfaction compared to 3% in the tramadol group (p<0.001).
Conclusion: Ultrasound-guided peripheral nerve block provides superior pain relief, better hemodynamic stability, and higher patient satisfaction compared to intravenous tramadol for managing acute pain in lower limb fractures. This method is recommended for effective pain management in emergency settings.
