Postoperative cognitive dysfunction: Preventive measures and treatment approaches in elderly patients

Abstract: Postoperative cognitive dysfunction (POCD) is a common complication in elderly patients following surgery, particularly those undergoing major procedures. The incidence of POCD is influenced by factors such as age, comorbidities, anaesthetic agents, and surgical trauma. This disorder can manifest as a decline in memory, attention, and other cognitive functions, which may persist for weeks or even months post-surgery. POCD can significantly impair the quality of life and increase the burden on both patients and healthcare systems. Preventive measures, including pharmacological interventions, non-pharmacological strategies, and perioperative care, have been proposed to mitigate the risk of POCD. Several studies suggest that the use of specific anaesthetics, such as total intravenous anaesthesia (TIVA), may reduce the incidence of POCD compared to inhalational agents. Additionally, perioperative optimization of comorbid conditions, such as hypertension and diabetes, may improve cognitive outcomes. Postoperative treatment strategies for POCD focus on cognitive rehabilitation, pharmacological interventions, and neuroprotective therapies. Cholinesterase inhibitors, N-methyl-D-aspartate (NMDA) receptor antagonists, and anti-inflammatory drugs have shown promise in treating POCD, although their effectiveness remains under investigation. Cognitive training and physical rehabilitation are also critical components in enhancing recovery. The purpose of this paper is to review the current literature on the prevention and treatment of POCD in elderly patients. By understanding the pathophysiology and identifying risk factors, healthcare providers can better manage this debilitating condition. The aim is to highlight effective strategies for reducing the incidence of POCD and improving recovery outcomes for elderly surgical patients.
