Tailoring analgesic strategies for high-risk cardiovascular patients undergoing non-cardiac surgery

Abstract: The management of analgesia in high-risk cardiovascular patients undergoing non-cardiac surgery poses significant challenges due to their heightened susceptibility to complications such as ischemia, arrhythmias, and hemodynamic instability. The administration of analgesics in these patients must strike a balance between effective pain relief and the potential for adverse cardiovascular outcomes. This article reviews the current strategies for tailoring analgesic regimens in high-risk cardiovascular patients undergoing non-cardiac surgery, with a focus on multimodal pain management strategy and careful pharmacologic selection. Non-opioid analgesics, such as acetaminophen, nonsteroidal anti-inflammatory drugs (NSAIDs), and regional anaesthesia, are evaluated in the context of their cardiovascular safety profiles. The role of opioids, despite their potential for cardiovascular depression and respiratory complications, is also discussed, particularly in cases where non-opioid options are insufficient. Additionally, advances in perioperative medicine, including preoperative assessment, intraoperative monitoring, and postoperative pain management, are explored in detail. The article highlights the importance of individualized approaches, guided by patient-specific factors, including the presence of comorbidities, medication regimens, and surgical type. It also emphasizes the need for a multidisciplinary approach, incorporating cardiologists, anaesthesiologists, and surgeons, to optimize outcomes. Key factors influencing the choice of analgesics and pain management techniques are presented, alongside recommendations for improving perioperative analgesic strategies. Ultimately, this paper aims to provide healthcare professionals with evidence-based guidance for optimizing pain management in this vulnerable patient population, ensuring both pain relief and the preservation of cardiovascular health and stability.
