Cardiovascular and thoracic anaesthesiology: Modern approaches to managing high-risk cardiac patients

Abstract: Cardiovascular and thoracic Anaesthesiology plays a critical role in managing high-risk cardiac patients, who often have complex co-morbidities, making perioperative care challenging. Advances in anaesthetic techniques and patient management have significantly improved outcomes for individuals undergoing major cardiac and thoracic surgeries. This review highlights modern approaches to Anaesthesiology for these high-risk patients, focusing on preoperative assessment, intraoperative monitoring, and postoperative care. Key tools like the Revised Cardiac Risk Index (RCRI) and EuroSCORE are vital in identifying high-risk patients and guiding anaesthetic management. Additionally, advanced monitoring techniques, such as transoesophageal echocardiography (TEE) and individualized anaesthetic protocols, have become integral to achieving optimal outcomes. Postoperative management strategies, including early extubation, multimodal analgesia, and minimally invasive techniques, are pivotal in reducing morbidity and accelerating recovery. Recent studies have highlighted the importance of early extubation, multimodal analgesia, and the use of minimally invasive surgical techniques, which contribute to reduced morbidity and faster recovery times. This review also emphasizes the role of anaesthesiologists in a multidisciplinary approach, collaborating with cardiologists, surgeons, and intensivists to provide comprehensive care. The hypothesis underlying modern anaesthesiology in cardiac surgery is that tailored, individualized anaesthesia can reduce complications and improve recovery, enhancing long-term outcomes for high-risk patients.
