Anaesthetic management of morbidly obese patients undergoing video-assisted thoracoscopic surgery: A case report

Abstract: Morbid obesity presents significant challenges during anesthetic management, particularly in thoracic surgeries such as video-assisted thoracoscopic surgery (VATS). This case details the perioperative care of a 61-year-old morbidly obese woman undergoing VATS wedge resection for lung nodules related to Hodgkin's lymphoma. Key concerns included complex airway management due to anatomical alterations, compromised respiratory mechanics, and altered pharmacokinetics of anesthetic drugs. A multidisciplinary approach ensured careful preoperative optimization, advanced airway planning with ramped positioning and video laryngoscopy, lung-protective ventilation using pressure-controlled volume guaranteed mode, and intraoperative hemodynamic monitoring. Desflurane was selected for its favourable pharmacokinetics. Postoperative care emphasized early mobilization, high-flow nasal oxygen, multimodal analgesia, and close respiratory monitoring to prevent complications such as atelectasis and hypoventilation. The patient recovered uneventfully without experiencing any respiratory compromise. This case highlights the importance of individualized perioperative strategies and vigilant monitoring to ensure safe anesthetic outcomes in morbidly obese patients undergoing complex thoracic procedures.
