Perioperative Kounis syndrome presenting as ventricular tachycardia and acute coronary event: A rare case report

Abstract: Kounis Syndrome, also known as allergic angina or allergic myocardial infarction, is an under-recognized condition in which acute coronary syndrome is triggered by allergic or hypersensitivity reactions [1, 2]. We report the case of a 48-year-old female who developed sudden chest discomfort, dyspnoea, and ventricular tachycardia immediately after receiving premedication with glycopyrrolate and ondansetron prior to surgery under general anaesthesia. The episode was accompanied by hypotension, elevated cardiac biomarkers, and dynamic ECG changes. Coronary angiography performed subsequently revealed normal coronary arteries, suggesting Type I Kounis Syndrome. The patient was managed successfully with corticosteroids, supportive therapy, anti-arrhythmic medications, and acute coronary syndrome protocol. This case highlights the importance of recognizing Kounis Syndrome as a potential cause of perioperative anaphylaxis presenting with acute coronary events, where timely diagnosis and coordinated multidisciplinary management are crucial for a favorable outcome.
